About Raqeem Bot — Clinical Assistant

What's New ? Raqeem Bot 2.29.2: what’s new New: Medication refill. A button beside Repeat Order on the Orders page opens a panel with two tabs. Last visit: it lists the Pharmacy items of the patient’s latest earlier visit with medicines, never laboratory orders. Each one comes with its indication, frequency, dose, duration, refills and quantity, kept within Raqeem’s own limits for that drug. By group: your usual medicines under Diabetes, Hypertension, Lipid lowering, Hypothyroidism, Vitamin D and Miscellaneous, plus an “Any other medicine” box. Every medicine is found in Raqeem’s own drug list. Adding and saving: Add puts the medicines into Raqeem’s own order grid, and Raqeem’s own medicine form opens for you to check. Save all presses Raqeem’s own Save. The panel then shows the Wasfaty ID when Raqeem has it. Nothing is ordered until you press those buttons, and the extension stores nothing. Rules and defaults built in Metformin: it follows the patient’s eGFR. ACE inhibitor and ARB: the two are never added together. Refills: they start at the most Raqeem allows for that drug. Typed quantity: a quantity Raqeem doesn’t calculate, such as a tube of ointment, starts at 2. Hard day limits: a medicine can carry its own, for example paracetamol at 2 days. New Vitamin D group 50000 IU capsules: once weekly for 8 weeks, no refills. 1000 IU capsules: once daily for 28 days, with refills. 2800 IU/mL drops: 1 day, capped, with 2 bottles. Added to Miscellaneous Omeprazole 20 mg: once daily, starting at 14 days. Paracetamol 500 mg: twice daily if needed, 2 days at most. Fixes in this build First click: the first click on the Medication refill button now opens the panel. It used to do nothing right after the page loaded, because Raqeem’s own invisible overlay was catching it. Order grid review: it now names the route (“Oral”) instead of showing an internal id. This update adds a Productivity panel to the OPD Worklist and widens the mammogram referral. The panel sits above the Registered, In progress and Discharged columns. It shows your registered patients, patients per hour, discharges against a daily target of 31, and your week (Saturday to Friday) against a weekly target of 144, added over the primary care centres you work in. Both targets are editable. Colours show how you are doing, the busiest and furthest-from-target figures flash, and an On leave switch turns the colours off. Counts are read through Raqeem’s own visit search, and only totals are kept in your browser. The referral tab now carries the pink breast cancer ribbon and asks where to send the patient: Dammam Medical Complex, another hospital from Raqeem’s list, or a primary care centre in any region. Centres are listed only if they have a mammogram clinic, with your registered ones first and a search by name or region. The run types Mammogram into Consult To Department and takes the one department Raqeem lists. Smaller fixes make the referral wait for Raqeem’s lists to load and keep the panel’s text readable in narrow tiles. 1. Laboratory Laboratory review: One click gathers the lab history since 2021 into one popup. Each result shows its unit, mg/dL conversion, reference interval, colour-coded status and trend. Critical shows red only when the lab itself flags it. You can filter, retry a failed read, and Export PDF. The PDF names the patient on every page. LDL-C is judged against the 2026 ACC/AHA goal for the PREVENT risk, not the lab’s population range. HbA1c and similar references: a result is judged by the lab’s own “Normal” line, and Raqeem’s HTML tags are removed from the display. eGFR sits under the vitals (CKD-EPI 2021). It shows the KDIGO stage G1–G5 in a coloured label, and is red and flashing at 30 or below. It appears within a second or two, with a “…” until the whole history confirms it. It gives no estimate when the data is unusable, and says why. PREVENT risk: 10-year (30-year for ages 30–59). Diabetes, smoking and treatment come only from your answers. 2. Tabs at the ICD 10 heading They run in this order: obesity, screening, chronic illness, referral. Obesity: picks E66.3, E66.91, E66.92 or E66.93 from the documented BMI. Screening: offers the five screenings that fit the patient’s age and sex, shows when each was last done, flashes a lab not ordered in a year, and enters the code on a press. Chronic illness (new): E11.9, I10 and J45.9, each with a note builder. Referral: for women 40–75, one press enters Z12.3, makes the mammogram Referral Out, fills the form, signs it and saves it once. 3. Chronic illness note builder (new) Text goes into Raqeem’s own Chief Complaints and Present Illness boxes for you to read first. Raqeem saves the box when you click away. Chief complaint chips: New Visit, Medication Refill, Follow up Visit. Diabetes: Pastes the last HbA1c and lipids with their dates. Suggests Controlled only when HbA1c is 7.0% or lower and LDL-C is below 2.6 mmol/L, both from the past year. You press the line you choose. Asthma: a scored Asthma Control Test with a suggested control status. It isn’t offered under 12. Hypertension: Controlled or Uncontrolled, added by hand. Medicines: your diabetes list plus an Insulin tab (Aspart or Lispro, Glargine or Toujeo), your hypertension list, and the starter asthma list. Each has dose and timing. 4. Forms and visit helpers M-CHAT (ages 12–36 months): fills the low-risk answers and saves only after Raqeem shows Total Score 0. Vitals: blood pressure, temperature and oxygen saturation from up to three earlier visits, with trend arrows. 5. Workflow Alerts: closes pop-ups with their own Close button (never “Mark as viewed”), and can open EMR instead of the Alerts page. CNF: completes a “called, not found” patient: Z53.9, the four note sections and Medical Discharge. It never enters vitals or starts a consultation, and it stops and quotes Raqeem on anything unexpected. Macros: record, save, export and replay navigation routines. Typed values are off by default.
