Capnolog Autofill for the ACGME Case Log Capnolog is a case-logging app for anesthesiology and surgical residents and fellows. This companion extension takes the cases you logged in the Capnolog app and prefills the ACGME Case Log form with them — in your own ACGME account, in your own browser — so you review and Submit instead of retyping. It is prefill-only. It never presses Submit or any other save control. You check every entry and submit it yourself. WHAT IT DOES • Prefills the ACGME Case Log form from cases you logged in Capnolog. • Pulls your unfiled cases from Capnolog's de-identified case sync once you pair it with the one-time code shown on your iPhone's Log tab (case sync on, which is the default). There is no app account and no ACGME login inside the extension. • Picks the exact Site and Supervisor (attending) options the ACGME form expects, and keeps your program's site and attending lists in sync with the app so nobody has to email a setup file around. • After you press Submit, it notices the case was filed and marks it filed in the app on every device, so a case is never entered twice. • Optional one-time history import: reads your own previously-logged cases from your ACGME record so your Capnolog counts start where your training actually is. • Optional form-structure recon: if you turn it on, it reports the structure of the form (field names, ids, and option labels — never the values in them) so the fill can be repaired the day the ACGME changes its page. WHAT IT NEVER DOES • Never presses Submit or Save. Never enters a case on its own. • Never touches your ACGME username, password, cookies, or session. They never leave your browser — we never receive them, never store them, and cannot sign in as you. • Never reads any page other than the ACGME Case Log site (apps.acgme.org). It keeps no browsing history and does nothing on the ACGME sign-in pages. • Never carries patient information. Capnolog cases are de-identified by design (case type, ASA class, age band and similar); the app's encrypted private note never reaches the extension. • Never uses anything read from the ACGME for statistics, research, or product analytics. Cases imported from your ACGME record are excluded from Capnolog's anonymized statistics. WHAT LEAVES YOUR BROWSER (only to capnolog.com — nowhere else) • The de-identified cases you chose to sync, and the fact that a case has been filed. • Your program's Site and Supervisor option lists as they appear on the form (no patient information). Entries that two residents independently report become the shared program roster. • If you run the history import: your own previously-logged ACGME entries, de-identified into Capnolog's categories, with the site and supervisor labels and the ACGME case number you certify — held only so your phone can pull them into your log, excluded from every statistic, and removable on request. • If you opt into recon: the structure of the form, never its values. • Zero-PHI diagnostics — counts, timings, and halt codes that say where a fill stopped. No case content, no page text. Deleted automatically after 30 days. • A receipt of the terms version you accepted or declined, tied to your device identity, never your name. The full description of every item, how long we keep it, and how to delete it: capnolog.com/privacy#extension BEFORE IT RUNS The first time it loads on an ACGME page, the extension shows Capnolog's terms and asks you to agree. Until you do, it does nothing on the page. Choosing "Not now" leaves it paused — you can still log by hand, and you can reconsider later. Please read this part. The ACGME's own terms of use restrict automated access to its systems. You use this extension on your own ACGME account and at your own responsibility, and the ACGME may act on an account it believes breaks its rules. If you are unsure whether the extension fits the rules that govern your access, don't use it — enter your cases by hand. Capnolog may pause or turn off the extension at any time, including if the ACGME asks us to; when it is unavailable you can always export your log and enter cases by hand.
